Default rotor damage 97 jetta

(97 Jetta) I noticed a pretty terrible noise while stopping for the past few days, and today i finally had a chance to take a look to see if i could spot any visual damage to anything. I found that my rotor has about a half inch wide ring of wear along the complete outer edge, as well as another ring of wear along the little bump out located in the center of the rotor. attached is a picture of where the wear is (in red), if my words arent helping. My question is, does anybody have any ideas what would damage my rotors only in these two specific areas? its only on the front drivers side wheel, the passanger side appeared to be ok.

more than likley just worn out pads
or a loose caliper (not very likley)
